A competitive nomination submission should include these items:

At least one letter of reference/recommendation
Nominee’s resume/career summary
Websites, articles, supplemental information describing nominee’s impact

Incomplete nomination forms will not be considered. Please refer to the awards webpage for awards information and general eligibility requirements.
