Elliott School Alumni Icon Awards Nomination Form

Thank you for taking the time to nominate a distinguished member of our Elliott alumni community. Your input helps us recognize those who exemplify excellence and make significant contributions to their fields, their communities, and the institution.

A competitive nomination submission should include these items:

  • At least one letter of reference/recommendation
  • Nominee’s resume/career summary
  • Websites, articles, supplemental information describing nominee’s impact

Deadline for Nominations:  February 22, 2026, 11:59pm ET.

Incomplete nomination forms will not be considered. Please refer to the awards webpage for awards information and general eligibility requirements.

Please provide as much concrete evidence of achievements, impact, leadership, and character—the elements that make for a winning nomination. 

Describe the nominee’s specific achievements, successes, and the tangible impact they have made in their chosen field, community, or for the institution. (Minimum 200 words, Maximum 500 words)
Explain how the nominee demonstrates outstanding leadership, character, integrity, and dedication. What sets them apart? (Minimum 100 words, Maximum 250 words)
Attach nominees resume/career summary, a letter of recommendation, and other supporting documents. Please ensure attachments are in PDF format and the file is no more than 10 pages long.
